1. Pharmacodynamics & Mechanism of Action
This sub-silo bridges mitochondrial-derived peptides (MDPs), inner-membrane cardiolipin stabilizers, and multi-receptor incretin agonists to remodel cellular bioenergetics, insulin sensitivity, and adipose partitioning.
- SS-31 (Elamipretide;
D-Arg-Dmt-Lys-Phe-NH2): Cell-permeable aromatic-cationic tetrapeptide that concentrates inside the inner mitochondrial membrane (IMM) independent of mitochondrial membrane potential ().- Cardiolipin Electrostatic Binding: Selectively binds cardiolipin—the signature conical phospholipid of mitochondrial cristae. In aging and ischemia, cardiolipin undergoes peroxidation and converts Cytochrome c from an electron carrier into a destructive peroxidase, collapsing cristae curvature and opening the mitochondrial permeability transition pore (mPTP). SS-31 shields cardiolipin, restores Electron Transport Chain supercomplexes (I–III–IV), halts mitochondrial proton leak, and increases ATP synthesis ( ratio) within in aged human and rodent muscle.
- MOTS-c (
MRWQEMGYIFYPRKLR): 16-amino-acid Mitochondrial-Derived Peptide (MDP) encoded by a short open reading frame within the mitochondrial 12S rRNA gene; acts as an endogenous “mitokine” and exercise mimetic.- Folate–AICAR–AMPK Axis: Inhibits the folate-methionine cycle at de novo purine biosynthesis, causing a accumulation of endogenous AICAR (5-aminoimidazole-4-carboxamide ribonucleotide), which directly activates AMPK and GLUT4 translocation in skeletal muscle.
- Retrograde Mitonuclear Translocation: Under metabolic or oxidative stress, MOTS-c translocates from mitochondria into the cell nucleus in an AMPK-dependent manner, interacting with NRF2 and ARE transcription factors to regulate nuclear stress-response genes.
- Multi-Receptor Incretins (Semaglutide, Tirzepatide, Retatrutide) & 5-Amino-1MQ:
- Semaglutide (GLP-1 RA): Mono-agonist at GLP-1R; enhances glucose-dependent insulin secretion, suppresses glucagon, delays gastric emptying, and acts on hypothalamic POMC/CART and brainstem nucleus tractus solitarius (NTS) satiety circuits while suppressing neuroinflammation.
- Tirzepatide (GLP-1 / GIP Dual Agonist): Biased co-agonist with relative preference for GIPR over GLP-1R; GIP agonism in white adipose tissue enhances lipid buffering and insulin sensitivity while central GIP receptor activation suppresses GLP-1-induced nausea.
- Retatrutide (LY3437943 — GLP-1 / GIP / Glucagon Triple Agonist): Adds Glucagon Receptor (GCGR) agonism, directly increasing resting energy expenditure, hepatic -oxidation, and clearing hepatic steatosis (MASLD) by .
- 5-Amino-1MQ: Small-molecule selective inhibitor of NNMT (Nicotinamide N-Methyltransferase) in adipocytes; prevents NNMT from wasting and as 1-methylnicotinamide, raising intracellular SIRT1 activation.
2. Pharmacokinetics & Sequencing Architecture
| Compound | Primary Target | Elimination | Clinical Bioenergetic Role |
|---|---|---|---|
| SS-31 (Elamipretide) | Inner Mitochondrial Membrane Cardiolipin | (plasma); prolonged IMM retention | Phase 1 Structural Repair: Rebuilds cristae and ETC supercomplexes before driving AMPK flux |
| MOTS-c | AICAR / AMPK & Nuclear NRF2 | (plasma); AMPK activation | Phase 2 Metabolic Flux: Exercise-mimetic GLUT4 uptake, fatty acid oxidation, endurance |
| Semaglutide | GLP-1R | () | Appetite suppression, glycemic control, cardiovascular/renal MACE reduction |
| Tirzepatide | GIPR + GLP-1R | () | Superior insulin sensitization & visceral fat loss with lower GI side effects per % weight lost |
| Retatrutide | GIPR + GLP-1R + GCGR | () | Adds hepatic GlucagonR thermogenesis & liver fat clearance |
3. Dosing, Cycling & Synergies
- Sequential Mitochondrial Protocol (SS-31 MOTS-c):
- Why Sequence: Driving AMPK/metabolic flux with MOTS-c in aged or damaged mitochondria with degraded cristae can increase ROS if the Electron Transport Chain is uncoupled.
- Step 1 (Weeks 1–4 — Structural Cristae Repair): SS-31 (Elamipretide) SubQ in the morning (with precursors / CoQ10).
- Step 2 (Weeks 5–8 — AMPK & Mitokine Activation): MOTS-c SubQ 2–3x/week administered prior to Zone 2 or HIIT aerobic training.
- Incretin Longevity / Micro-Dosing Protocol:
- Clinical Obesity Dosing: Tirzepatide ; Retatrutide .
- Euglycemic / Anti-Inflammatory Micro-Dosing: Tirzepatide twice weekly or Retatrutide , paired with mandatory resistance training + protein + Creatine & Phosphocreatine System to prevent lean skeletal muscle loss.
- Contraindications & Biomarkers to Monitor:
- Contraindications: GLP-1/GIP/GCGR agonists are contraindicated in personal or family history of Medullary Thyroid Carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), history of pancreatitis, or gastroparesis. MOTS-c consumes folate-cycle intermediates—always co-supplement with L-5-MTHF (Methylation & B-Complex) to prevent folate depletion.
- Biomarkers: Fasting Insulin & HOMA-IR, HbA1c, Serum Lipase & Amylase, Calcitonin, DEXA Appendicular Lean Mass Index (ALMI), and Resting Heart Rate / HRV (GCGR/GLP-1R sinoatrial node receptors typically raise RHR by ).
4. Key Scientific Evidence Summary
- MOTS-c Discovery & Exercise Capacity: Lee et al. (2015, Cell Metabolism) and Reynolds et al. (2021, Nature Communications) demonstrated that MOTS-c activates the AICAR-AMPK pathway, prevents age-dependent and high-fat-diet-induced insulin resistance, and doubles running capacity in aged mice while increasing human skeletal muscle MOTS-c expression by during exercise.
- SS-31 Human In Vivo Mitochondrial ATP Restoration: Siegel et al. (2013) andKaraa et al. / Campbell et al. (2019) showed using that a single dose of SS-31 (Elamipretide) in aged humans and mice rapidly restores maximal mitochondrial ATP synthesis () and coupling () to young adult levels within 1 hour.
- Retatrutide Phase 2 NEJM Trial: Jastreboff et al. (2023) published in NEJM () that Retatrutide achieved a record mean body weight reduction at 48 weeks (with of subjects on losing weight and resolution of MASLD liver fat).
